I tried converting each file separately, and it seems the problem is in the
attached pwa_otaPackages.dita
I'll attach the .profiles file as well, in case that helps; it's giving good
results with all the other files

It's been a while ... so here's a question to show I'm still happily using XXE
and exploring more things I can't quite do :-}
I have a DITA task ei_004.dita with the ID ei_24lb68 where one of the steps has
substeps; the substep has the id ecrDevice, and shows up (correctly) as 3a
