Luckily, I was also working with a smaller .bookmap and a set of updated files. I tried converting each file separately, and it seems the problem is in the attached pwa_otaPackages.dita
I'll attach the .profiles file as well, in case that helps; it's giving good results with all the other files in the .bookmap.
