Sorry I didn't provide all the information you needed in my first e-mail! 
though you've clearly managed to fill in the gaps.

Just by way of confirmation ... I'm now quite comfortable using the status 
attribute on a ph element to track changes, which is something I only need to 
do to highlight final differences between release -1 and release 1 of a 
document. The minor snag at the moment is, indeed, that I need to switch 
between .profile to see markup in the source and .ditaval to highlight the same 
markup in output. Some sort of 'unified option' that combined the two would be 
a nifty extension of XXE's already impressive functionality :-}

Highlighting new/deleted content while writing in XXE may be implemented by

I assume that you have done this.

A .profile file when associated to a DITA map, automatically creates a number 
of DITAVAL files (one per profile) allowing to *exclude* some contents from the 
PDF file generated using "Map|Bookmap -> Convert Document".

Currently, these automatic DITAVAL files do not support *flagging* some 
contents in the generated PDF file. They only support excluding some contents.

This is clearly a limitation which could be removed in the future.
Therefore you are 100% right in asking the above question. Thank you for asking
