Amazing ... even on a Monday morning, I've managed to remember something. By
converting each chapter as a separate .bookmap, I was able to locate the
problem topicrefs. Looking at the topics, I then realised my mistake: I'd
applied a <status> to a <row>.
Problem now solved!
