The problem is clear enough, and I was nowhere near suggesting a bug - what 
look like two identical <topicref> groups are behaving differently.

The answer, as you helpfully remind me, is almost certainly the "navigation 
links" setting. You'll allow a mere author to decide when his readers / content 
require family links?

Now to change the setting and offer the readers what they need :-}
