I think that's your problem. You're running too far on straight (untwisted) wires. If it is in PVC, can you run another line of CAT3 in there and get proper twisted pair? I'd bet things would clear right up then. I had very similar problems one time in a client's house, until I found he had wired the house with thermostat wire. Rewired with UTP and all was well.
