I only get something changing the Zoom %. Changing the Screen DPI has no

I imagine you output looks fine and this is a problem with display only.
Have you installed the Maths fonts (Bakoma)?
These help Lyx display math properly.

reference numbers appear in the dvi output, but the footnotes

Yes, this seems to be exactly what I was looking for.
Using the soul package and this command means I no longer have to
individually deal with LyX's problem in hyphenating underlined words.
