That was likely the problem. We ended up reconfiguring without MHR since it's
for non-commercial use anyways. Leaving MHR out fixed the issue for our use

scanner+clamscan+spamassassin, which is successfully handling 60,000+
messages per day. However, a small number of messages are never making
it past qmail-scanner. Below is the relevant portion of the
qmail-queue.log for one such message:

use a proven setup that has worked fine on other systems (single proc),
but does not work on the new (dual proc) server.

server pool.
I have used the same install procedure that worked before with Qmail,
Spamassassin, Clam Antivirus, and Qmail-Scanner. I decided to upgrade
to the latest version of Qmail-scanner for this server as a test
exercise.
