I have a large installation of qmail+qmail-
scanner+clamscan+spamassassin, which is successfully handling 60,000+
messages per day.  However, a small number of messages are  never making
it past qmail-scanner.  Below is the relevant portion of the
qmail-queue.log for one such message:

The only relevant items I found in the archive for the "no sender and no
recips" line were a patch related to Perl 5.8.0, but I am running 5.6.1.

I have scrutinized the failing messages line by line and can't find
anything in them that is out of the ordinary.  Suggestions welcome!
