by all the DocBook stylesheets generating HTML and XHTML I've used, including the profiling stylesheet generating multi-page XHTML5.

Okay, I'm sorry, it's my fault.

I changed the generated file's extension from "xhtml" to "html", assuming it would work as hybrid/"polyglott" (X)HTML. This messed up the markup.
