To respond to both you and Shane, here is the output of the
qmail-showctl program, and a copy of the exact lines in the assign file
:

The *only* thing I see different about the way this domain is set up and
other domains (that do work) is that this domain "fell" over into the
morercpthosts file..  Otherwise, everything else seems identical...
