I’ve run into issues with case sensitive HFS system drives on OS X within the last ~2-3 years. The issue, of course, was with Intuit and TurboTax. A case sensitive filesystem caused TurboTax to crash (on launch — I think — can’t remember). Wee! Formatting, reinstalling, and restoring data just to file taxes was aaaaawwweeeesome!
