Always use the default expected by OSX, which is case-insensitive.  Any time 
you stray from the beaten path, you introduce yourself to error conditions that 
the original software developers never bothered testing (or didn't test as 
thoroughly.)

I have often given this advice about the OS drive.  Less often, I have given 
the same advice about external drives.  Your situation almost proves it - 
although you haven't conclusively identified the case sensitivity to be the 
root cause of the problem, it is highly suspicious, as I've never encountered 
that problem on hundreds of systems I've supported over the years.
