Same problem... twice and always with WD Disks and a RAID (RAID0 2disks
WD 80GB and RAID1 1 WD 500GB and 1 Samsung 250GB) The first time I ran
the write-mode badblocks test and the result was NO errors and problems
happen more one time. The errors never was with the Samsung disk :S

In order to get around this on my system I had to put "insmod lvm" into
the grub startup, but now it is happening again and "insmod lvm" is
saying "file not found".

Am I correct in my interpretation of the description that you were able to
resolve this problem by:
Disabling SATA NCQ (native command queuing) :
echo 1 > /sys/block/sda/device/queue_depth

Forget to say in my previous message that the array was working
perfectly for around 1-2 months before the upgrade.
Also, first time it happend was soon after a reboot after upgrading the
kernel. The HD led was on non stop, could move the mouse but Ubuntu was
unresponsive.
