I am riding a 62cm Rambouillet with relatively hight handlebars.  I am using 
the largest Berthoud bag and it places the bag just below the bars.  I am using 
the spacer mounted decauler from Velo Orange.  I am using this on my tandem and 
Quickbeam as well.  My front rack is a Berthoud, don't care for the grey epoxy 
finish but the light mount is great.  
 
First determine where you want the top of the bag to be and measure the 
distance from the top of the front rack to this point.  That is the height bag 
you want.  All the decaulers I have seen need to be bolted throught the bag so 
you will need to make some holes in the material.  To determine where on the 
bag you need to mount this part, I set the bag on the front rack and mounted 
the other piece of the decauler on the bike and marked the bag where it all 
came together.
 
Rivendell bikes are not optimized for front loads.  I was very unhappy when I 
used my Boxy Baggins with a heavy load.  The Berthoud Bag along with the rack 
is much better I believe because it lowerd the load and brought it closer to 
the head tube.  The ostrich bag is low fairly deep so your load will be further 
forward.  The Berthoud bag is Tall and shallow and I think this results in 
better handling on my Rambo.
 
How tall the bag should be is up to you.  I use mine to hold a queue sheet so I 
wanted a fairly tall bag.  As someone else pointed out he wanted interupter 
levers so got a lower bag.
