So something is creating the files as qscand user but in roots group. Do you have any idea what this could be? Surely the only thing writing files there should be qmail-scanner?

There is only one place where that error occurs
Replace it with
&error_condition("owner of unpacked files (uid=$uid) doesn't match UID of Qmail-Scanner (uid=$effective_uid) - can't expect this to work. Fix whatever is creating files (like \"$filepath\") with uid=$uid");
...and then it'll tell you the actual filename that it didn't like.
Basically it is checking that "$filepath" is owned by "qscand" - and that check says it isn't.
