I really don't thik there is a bug with my system, I've sucessfully had qmail-scanner 1.22 working, it's just this new (1.23) release.

Permissions are fine here:

ClamAV is also running as "qscand". Is there anything else I should check my end before reporting a bug?

It's telling you it's a "bug" with your system!

Check the ownership of all the files under /var/spool/qmailscan/ -
everything under ./tmp and ./working must be owner "qscand" - otherwise you
have a broken install of something.
