As Chetan mentioned, nothing if you already have a BSNL phone. Just a simple one page form. Also, the persons who you submit the forms to know nothing about the tariff plans, so any queries there are useless. They are also pretty clueless about other details that an informed customer might ask, like download cap or bandwidth. Research on BSNL's website is advised before you visit the exchange.
