I still think it's a problem with the underlying text wrapping. I misunderstood your first message - I thought this was occurring with emails you were composing. If a client hard wraps a paragraph at 100 characters, and you mutt soft wraps that same paragraph at 78 characters, you'll end up with a bunch of lines with 78 characters, interleaved with lines of 22 characters.

The only solution is to have mail clients either never insert hard breaks (which can be problematic because not all receiving clients soft wrap properly) or to have all clients use format-flowed. Which they don't.

Unless I am mistaken, there is not much you can do.
