Using a large mallet, Jeremy Blosser whacked out:

I use set envelope_from (Furio, that needs a newer mutt, I suggest you upgrade
to the latest stable - 1.2.5). However, I use folder hooks to set the sender
(and procmail to filter mail into different folders where I use different
aliases). The alternates / reverse_name doesn't seem to work too well with
folder hooks.
