It intrigues me how a log from a crashed phone does not shows when the problem is happening.

Simon,

Yes, I totally understand.  That's exactly why I've often mentioned that this is a difficult problem to nail down.  People often have different settings, different model of iPhone, some using wired or wireless headphones and different methods of hanging up the call.  Then you compound this with some inconsistencies, and others never actually having the problem, and it's brutally difficult to fix.  Developers won't even know where in the code to look for the conflict until these sorts of things get figured out.

So what are people doing differently when hanging

Hi Anders,
Yes, I'm not really sure either.  I've only had it happen two or three times out of probably hundreds of calls since my update to iOS 13, whereas others have had it happen almost every time.

That work around would be okay, however if we turn off voice over
before we disconnect then the two finger double tap or magic tap
won’t work to disconnect the call. We would have to depend on the
other person disconnecting the call. And does one lose voiceover
if one just waits for the person at the other end of the call to
disconnect? I’ve been lucky so far and haven’t had any major bugs
    with IOS 13, touch wood.

You know, it just occurred to me Lloyd when you mentioned the
triple click press to get speech back, that I’ve been dealing
with speech loss since iOS 12, and my solution has always been
to use the triple click press, only I had to do it two or
possibly three times in a row to get speech back it seems to
me that people have tried it but I’ve only been doing it the
one time. I don’t know, just a crazy idea, but try doing it
twice even three times, in fact I’ve had to do it four times
in the past it’s like you have to it’s like turning speech off
and then back on again and then back on again once again for
good measure. If that makes any sense. I will say that when I
did lose speech, I still had the voice over Sounds just no
voice output. I don’t know if that’s the same condition that
we’re having here with this new version or not. Anyway, give
it a try triple click three times or even four if necessary
then give up on that one
