Hrm, Miasma could work as a more granular kind of system than cards. Maybe have Miasma go from 0 to 100, and minor punishments grant 0-25 Miasma, while more severe ones grant 25-50. You'd lose 10 Miasma each month, and depending on how much Miasma you've got, you'd be more or less "sick" with different crippling effects (Just the regular "less Agoran decision power" comes to mind).
