You need to find out what the IP address (external) is that is assigned to your friend. You use THAT address to connect to. Your friend should be able to link to his router and get his WAN address.
You MAY still run into a problem where the router has most ports turned off for safety and you may have to allow access through port 5900. SOMEONE had a neat little list of this setup (not the long list of instructions, just the routing table) last week, but I can't find it right off.
