Toilet in the OSM sense refers to the area where the activity can take place. It's generally tagged on a room or a building. A five star hotel lobby toilet or an outhouse shanty get the same tag, as long as they are open (under give access= rules) for the purpose of elimination.
The newish toilet:position tag takes care of the style of the actual facilities. Micromappnig tags are available for given gendered entrances. The "access=permissive" tag covers the toilets you can get to without formal permission. It's a tricky tag, but essential, given the prevalence of permissive toilet facility use. A tag like "access=destination" or "access=customers" covers cases like Museums open to the public for a fee, with a customers only restroom inside. --------------------------------- The toilet tagging itself is all working fine!
