Outlook 2000 can do rules on headers; Outlook Express, however, cannot. ...with specific words in the message headers.... is the rule type.

I thought not. If you choose to have some "[spam ...]" text prepended to the Subject, it will only be added if the message actually _has_ been flagged as spam (as defined by your threshold).
