I had a quick glance at the website, and wandered onto the page where he discusses why it's difficult for spammers to filter their lists for the addresses generated by sugarplum. That discussion suggests that some portion of the "random" domains in the addresses are deliberately designed to be real domains for which MX lookups will succeed.
I was looking at that also, but I got the impression it was simply an infinite monkeys type of thing. In other words, the components of the addresses are generated randomly, but the structure is valid, and there's always a slight chance that someone really does have the address [EMAIL PROTECTED]
There is also a mention of optionally mixing in your own tarpit domain(s).
