When someone subscribes, send them an email requesting confirmation (and include some large random number in the message, which they must use as a confirmation, so as to avoid people guessing what the confirmation code might be). Until they successfully confirm, do not send that email address _ANY_ other messages. Maybe give them one follow up "hey, did you miss the confirmation?" and that's it.

I personally consider "unconfirmed opt-in mailing lists" to be as bad as opt-out lists, and I have no conflicted feelings about blacklisting those people ... they may not be spammers, but they're only one layer of scum away from the bottom of the same barrel. Now, my hatred for unconfirmed opt-in lists probably comes from my home domain being 'domain.org', and all of the lists I get signed up for by random idiots around the net. But, frankly, not getting confirmation is just not doing 'due diligence'. If the list owner isn't acting responsibly (which includes seeking proper confirmation), they deserve to be blacklisted (personally and/or publicly).
