We run SA system-wide with about 15k customers. We don't offer opt-in/out, however. We're running on a dual-athlon 1900MP with Red Hat Linux 6.2 As for scalability, have a look at running spamassassin through mimedefang. In particular, mimedefang-multiplexor does an incredible job of cutting down on processing time (even compared to spamc/spamd).
