 I'm not Duncan, but... my source is that I've only gotten e-mails with both
sir and madam when it's been spam; but at the same time I know that esp. people
in the US like to use either sir and madam if they're writing something
business-related... and it's possible that they'd write both if they're
sending their e-mail to more than one person.
