In Exim it isn't even always a separate file.
Then there's the 'allow domains for which we are a valid MX' rule, which is quite common on large installations (surprisingly, spammers don't attempt to create a bogus domain to get around this kind of rule... if they ever did it would rapidly become a depreciated option!).
