Just the connecting host, I believe.

To answer Olivier's comment, exim can be configured to "warn",
"reject", "skiprelay", or "accept" (for RBL-style whitelist) based on
RBL.  I have my setup set to "warn" so that it merely adds the
X-RBL-Warning header and accepts the message as usual.  I then have a
filter that sticks messages with that tag into an "rbl" folder so I
can see how (mis-)effective it is.
