I just re-installed a clean Xubuntu 12.04 LTS and can confirm that both
"Speakers" and "Headphones" outputs work out of the box. It isn't able
to automatically switch between Speakers and Headphones, but switching
manually from Xubuntu's Volume Control app works.

My client side (with totem-gstreamer) is Karmic and the pulseaudio
server is Hardy. Getting exactly the same error when trying to play
stuff with totem (or totem mozilla plugin) when the network sink is
active. Mplayer, Xine and flashplugin all are nicely able to talk to the

Seems like I've lost my GPG key so no PPA packages for a while then...
Anyways the fix to this bug is libmtp 0.3.3 + mtpfs 0.9. I've already
created a bug report for updating mtpfs which does require a newer
version of mtpfs to even compile properly.

Mutiple file transfers appear to cause lib
I just built the newest versions of libmtp (0.3.3) and mtpfs (0.9). So
far every transfer I've tried via Nautilus has succeeded :) So the fix
to this bug seems to be getting the newest upstream versions of libmtp &
mtpfs.

The problem you were seeing could just be a problem with ZynAddSubFX
which doesn't really work that well with jack. It's jack connection
pretty often get's "zombified" and it just doesn't produce any sound
anymore. Of course in that case triggering notes using Zyn's own virtual
keyboard also fails
Do you get anything else on the logs before that error? And have you
tried to turn on debugging to see what's possibly causing the crash?
