standard layout, and can't figure out how to include include it in Lyx. I
found reference to a style file in the LyX and the LaTeX preamble section of the
User's Guide but I couldn't make out where to install the file.

As I mentioned before. The file that I have
attached runs into the footer section of some pages. I didn't recall doing
anything out of the ordinary, but given I am still experimenting there is always
the chance I took a wrong turn somewhere.

Debian GNU Linux 2.2 (not necessarily the same packages are installed!) and I
