"header" into the LaTeX code instead, it came out fine.

In the attached extract, I want a 'followVoice' line connecting beats 3
and 4, and no others. Depending on where I place "\set followVoice =
##t" I get either additional unwanted lines between other beats (bars 2
and 3), or no line at all (bar 4). What's going on? Is this intended
behaviour?

The problem is that we don't have enough people working on the
basic documentation -- how to change staffs in piano music, how to
change beaming, how to write an F-double-sharp -- such that we can
finish the basic stuff with

I think that we could enhance the section "A.7 lilypond-book templates",
like expanding the example with how to use a 'makefile' and how to input
chapters and figures. A minimal example like the one I just posted to
the list.

The fundamental thing to remember is this:
To use \lilypond or \begin{lilypond}, you need to write
a .lytex file. This *CANNOT* be done inside a .latex or .tex
file.
Use lilypond-book to translate from .lytex to .tex.

lilypond-book works quite well for such things; I'm using it for
my thesis. Please search the mailist archives from about one or
two months ago; we discussed this recently, and I posted a working
example with a makefile.
