I am now completely free of proprietary drivers and so far it seems to
have worked. I rebooted twice and it the problem was not there.
Im on 12.04, I can select which monitor but regardless of which one i
choose, the launcher is only placed in the primary one. This has worked
in the past when I had two different monitors, but now that I have two
of the same monitors it has stopped working.

I have very recently made a clean install of 16.04LTS with what appears

It is intermittent for me, and not immediately reproducible, however I
was able to reproduce it and grab some journalctl lines via SSH if it is

I can confirm this under Gnome as well. Starting a Wayland session, all
works fine. Logging in under Xorg results in this graphical issue for
both Firefox and Chromium.
Same GPU as the initial reporter, however I am on 22.04 and kernel
5.15.0 (stock HWE).

Can confirm this behaviour on Ubuntu 14.04. I'm using Firefox 39 with
only one addon (ublock origin) and all plugins either disabled or set to
ask for permission to run. Experiencing crashes in both Firefox 39 and
Firefox 40 Developer Edition, but Firefox 38 is fine.
