I agree with you, except for bike lanes and dooring; striping along the
rightmost part of a lane does at least remind motorists that cyclists are,
or may be, about. I don't see how cyclists can be doored, unless you are
talking about bike lanes to the *left* of parking lanes, which I would
wholeheartedly agree is a bad idea, unless there is very ample space,
crosshatched, perhaps, between car park lane and bike lane. But most bike
lanes, around here, are at the rightmost edge of the road -- no room for
car parking.

That apart, what really will make cyclists safer is, I truly believe, more
cyclists, more visible, more of the time. People are much more likely, I
think, to anticipate what they're used to seeing.

Patrick Moore, who used to cycle 9 miles across town on un-laned, major
artery, Central/Route 66 at rush hour, in the winter dark, in ABQ, NM --
and wouldn't do so as lightly these days!
