I have a 10 cm DD and 46 noodles on my 56 Samuel Hillborne. I have the stem raised about an inch, which puts the bars exiting the stem about an inch above the nose of my saddle. I I was supposed to get an 8 cm stem, there was a mixup, but in retrospect the 10 is more aesthetically pleasing to me as the 8 has this step below the extension while the 10 has no transition. I ride with TRP levers, interrupters and silver bar ends and I still probably have 5 different hand positions. This is my favorite and most versatile setup. If I didn't like the ergonomics of the TRPs so much I'd try Gevanelles and lose the bar ends which are level with my TT.
