The only thing that I think could be, is that qmail-inject is sending to "root" from "root", without a domain part. In this case qmail-inject should add the content of control/me or control/defaulthost to the sender and the recipient, and maybe there is some problem there... You can do some test (if you have time...) sending with qmail-inject some file with a mailformat, you can find some in you quarantine... qmail-inject -a -f root root < name.file.to.send And see what happends.
