I must have just left those off when testing a multitude of different

With the quotes in place I now get:

I even removed the RBLSMTPD variable:

:allow, RELAYCLIENT="",QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue.pl"

same problem. Any ideas?

The above implies that you need ="" after RBLSMTPD.

for example:
195.41.46.:allow,RBLSMTPD="",QMAILQUEUE="/var/qmail/bin/qmail-scanner-queue.
pl"
