Have you read the configure script? :-)

It simply uses the "strings" command to see if the phrase QMAILQUEUE shows
up in the qmail-queue binary - if it doesn't, then the patch hasn't been
applied.

I can't understand how 1.16 can be working if QMAILQUEUE doesn't show up in
the qmail-queue binary...
