what is so difficult to undestand?

if you are lacking knowledge that a matching PTR and HELO
should be one of the first things you have to care you are
not qualified to maintain a mailserver these days

if your IP is sending spam you will get blacklisted

if you have a dial-up IP you do not care about quality
nor have you any knowledge how e-mail these days is
handeled - MOST spam ist from botnets and these are
mostly infected clients on dial-up networks

if i get 500.000 spam messages each day on a small
server i have to filter them EFFECTIVE and not by
content-scanning, the most effective way is

* blacklists
* filter dialup ranges
* filter HELO hostnames
* filter hosts with no PTR

there may be some non-spam mails because ignorant and/or
uuneducated admins maintaining a mailserver, but that is
their problem because the 4 points above are EASY to fulfill
