I've removed KDE installation from packages, installed a fresh new one of
version 4.2.2, checking only kdegraphics in installer - thus only it and it's
dependencies are installed.
Opened the first pdf at hand and checked - all icons are fine. Checked the
package itself - all needed pics are there - KDEROOT/share/apps/okular/pics.
It's not packaging issue, for sure.
Note that all previous and even self compiled okular from trunk hod no such
bug. It's something with your system, installation or something similar.
Checked on Win XP SP3, MSVC build from installer, v4.2.2.
