I have a really old KDE-Windows install that I update periodically. My okular.exe got updated with the rest of KDE Windows on 2009-04-21. I can indeed see those tool icons in /KDE4/share/apps/okular/pics/ and it has same icons as in Kubuntu's /usr/share/kde4/apps/okular/pics, so I'm not sure why Okular on Windows didn't use them. Next time I'm in Windows I'll try running it under Process Monitor.
