I am still having no trouble—or, at least, no more trouble—than I was before, 
with the iOS app. But the issues persist, with a vengeance, on macOS.

And I don't know whether it's related, but I'd say a big part of the issue is 
indeed that VoiceOver makes it very hard to explore outside dialog elements. 
Apple believes in a utopian world where dialog elements only exist for the 
user's benefit; sadly, that's not the reality and screen readers need to help 
the user when they're stuck in an inaccessible element, even if this isn't what 
the web page designer wanted. Examples: newsletter signups, cookie consent 
banners, ads, etc. Especially when you've disabled script or used blockers to 
filter the requests that would be needed to hide the elements.
