Take a look at your Safari preferences menu and select the tab button  
on the toolbar (Command-comma to open preferences, interact, navigate  
to tab button and press (VO-Space), then look at the tab settings.)   
The command sequences seem to depend on how you set the two checkboxes.

1.If you have the box checked for
   ⌘-click opens a link in a new tab
then the default option is to open the page in a new tab instead of a  
new window when you Command-Click on a link, and you have to use  
Command-Option-Click to open the link in a new window.  If the box is  
unchecked, these definitions are reversed, with the default to open in  
a new window with Command-Click, and the addition of the Option key  
needed to open in a new tab.

2. The second check box determines whether the new tab or window is  
made active (i.e., receives focus), so if you check the box for:
  When a new tab or window opens, make it active
then using Command-Click or Command-Option-Click will not only open a  
link in either a new window or tab, it will also move your focus there.

Assuming that you want to open links in a new window and have it  
receive focus, you would uncheck the first checkbox and check the  
second:
⌘-click opens a link in a new tab unchecked check box
When a new tab or window opens, make it active checked check box

Then the following sequences will work:
Command-Click: Opens a link in a new window, and makes it the active  
window.
Command-Shift-Click: Opens a link in a new window.
Command-Option-Click: Opens a link in a new tab, and makes it the  
active tab.
Command-Option-Shift-Click: Opens a link in a new tab.

Incidentally, there's something weird about the way VoiceOver is  
navigating to and announcing these options on this menu, and it seems  
to have to do with the use of a symbol for the Command key.  But the  
above definitions seem to work.

I can't imagine any VoiceOver users wanting to open new tabs or  
windows without making them active, so everyone should check the box  
for the second checkbox option:
When a new tab or window opens, make it active

Then, the only question is whether you want to open links in a new tab  
or in a new window.  I usually use new windows, and use command-accent  
to switch between windows, so I uncheck the first checkbox to open  
links in a new tab, Command-Click on the link, then close the window  
with Command-W when I'm done.
