I have (or Michael, rather): a single choice box containing what are
now "type" entries, plus all the branch names available in this
document (an editable list saved in doc header).

blind: old-fashioned note, not output, just for the author.
greyedout: output as grey text
petite: small(er) print ("intermezzo" would be better name, functional
not visual)
comment: outputs verbatim's "comment" environment. Functionally same
as "blind", but done through LaTeX.

currently impossible. branch name and inset type are different
variables. Under the new system we will have to handle that (disallow)
somehow.
