Regarding my case patches of a few weeks ago:

How very embarrassing. My last fix to the file, I forgot to reload LyX before testing. As a result, the \mediumskipamount I have instead of \medskipamount causes failures. Please accept my apologies, and a corrected patch. I've also tried to make a patch against the trunk, as requested.

In response to Paul Rubin's comments elsewhere: yes, this makes only formatting changes. It makes the formatting much closer to what I saw in some journal articles. I do have the "Case #" indented, which may not be quite correct, according to some style manual. However, it's good enough to be useful, which (at least for me) the previous version was not.
