Sounds like they haven't changed in two years.  We ran a 30 day test 
with them and dropped them too.
Good:
byod- good rates
excellent control panel, and we were able to use all the features of the 
sipura
excellent audio, when it did work
Did have a $17 setup, and unconditional guarantee (30 day 100% refund)
Bad:
Way too many times we had no dialtone when going off-hook (DFW softswitch)
Calls went to vm rather that ring
Never could port numbers
Could not support multiple lines (on sipura)
Added fees

Some of the dialup isps are converting their nets to voip, you might 
check out joi, we had many customers use them as backup isp.
voip rates look good but haven't tried them yet, on my list.  they have 
a $4.95 /mo promo- (the promo # is their generic #, I have no connection 
to them whatever)
I also did get the missing numbers: setup was $30, shipping $15, monthly 
fees $1 (from memory, couldn't find my email)
