You can't solve the problem unless you know what you are dealing with. Have you tried placing a standard telephone (mechanical ringer) across the line to check for ringing characteristics? If there is a problem when connected directly (no other loads) it is your telco's problem. Be sure to check the phone at the office before you leave for the site.
Where is the customer located? Maybe someone on this list is nearby and wouldn't mind taking a fresh look at it. Have you tried connecting the KSU directly to the incoming line (no other wiring involved)? Additional loads on the line can overload the CO ringing generator. We have had several cases locally where the lack of incoming calls, or reliable ringing was caused by a faulty Ring Generator at the CO. You need a voltmeter that will read accurately at 20 Hz. Take all loads off the circuit and measure the ring voltage for an incoming call. You could also use an oscilloscope, but remember you are dealing with a balanced line, so you may need two probes and a differential input.
