I have had this happen a couple of times on a TDA100. I would reset the card 
by taking it ous and back ins, and the CO would clear.  Has not happened for 
some time. Also on this same system I had 2 DLC16 cards go bad. It seems 
that once those cards were replaced, the CO problem also stopped.
BTW, just yesterday, I had another 16DLC failure. I thought I seen the last 
of these failures.
